  2. Has a signed DNR
  3. Prognosis of 3 months or less

These are fluid and not always set in stone. Call the director to discuss if you feel it is questionable whether your patient may or may not meet these standards or expectations.

  1. Under 200lbs
  2. Non-ambulatory (not up ad-lib)
  3. Does not require injections, such as insulin
  4. Will adhere to our non-smoking policy

  1. Meet the minimum eligibility requirements
  2. Be in need of a place to receive hospice services
  3. Need a 24/7 caregiver
  4. Agree to our safeguards and standards
